The employee performance appraisal form is one of the performance management tools used during the employee performance appraisal and planning stages of the employee performance management process under the organization’s performance management system. . The Employee Performance Evaluation Form has 2 components

1- The Objective Record Sheet (ORS) – This employee performance evaluation form is completed by the employee throughout the performance planning phase at the beginning of the year and serves as the basis during the quarterly performance evaluation and the semi-annual review. It comes in excel format with two main segments, the first being Objective Indicators that are made up of measures in the aspects of quality, quantity, time and cost. At the other end of each measure are the corresponding ratings of excellent, good, fair, and poor. Another important segment is that of Activities that covers the vital activities that will be carried out; completion period; cost; necessary support and; the source of support. After critical consideration, the employee and immediate supervisor sign the ORS. An ORS must be achieved for each KRA and goal/strategy.

2- The Individual Operating Plan (IOP) – Similarly, the staff must complete this employee performance evaluation form in the planning stage, but it must be used in its entirety in the quarterly and periodic review. In addition, it comes in excel format consolidating the entries of the ORS. The form has 3 main segments, the first of which is Performance with a corresponding weight of 90%. These are the supported institutional strategy/goals; Key Result Areas (KRAs); weight per KRA; individual goals; classification for ORS and; the final grade. The second is the Behavior Segment that represents 10% of the weight. It involves both the self-assessment of the employee and that of the immediate supervisor. Opposite them are entries about demonstrating the organization’s core values; feedback from customers and co-workers and; Average score. The third and final segment of the form is the Overall Rating, which includes the overall weighting; formula; weighted rating and; the overall numerical rating. As usual, the IOP is signed by the employee and approved by the immediate supervisor.
